GRANT AWARDED - Carnforth Integrated Care Community

The 3R Foundation recently awarded £250 to Carnforth Integrated Care Community which they used towards making up some Christmas Hampers which will be gifted to some of the patients the ICC and partners have worked with during 2023 and that we feel would benefit from receiving a Christmas gift.

Integrated Care Communities (ICCs) exist across the Morecambe Bay area, the ICC are made up of teams of health and care workers, voluntary organisations and wider partners who are working together to improve physical and mental health outcomes, promote wellbeing, reduce health inequalities and focus on wider determinants of health across the population in Morecambe Bay.

Carnforth has a small Integrated Care Community; with a Care Coordinator who works with older patients in the local area to ensure that people have access to services and local resources to help make life easier as they age. They seek to reduce hospital admissions, empowering people to discuss and document the things that are important to them and to stay living independently. Each Christmas they seek funding from local groups to enable them to prepare and distribute Christmas hampers to some of the individuals that they have worked with during the year.

Carnforth Integrated Care Community have prepared these Christmas Hampers for the last 3 years. Acknowledging the cost-of-living crisis and the need to ensure that the basics are provided first, the team have created hampers with more longevity, focussing on food, drink and other items that will help people to stay warm and well during the Winter.
